As a Founding Member, your consultation fee is completely waived — giving you direct access to care without any upfront cost. Note: Travel Consult/Vaccination consultation fee still applies.

Membership is billed monthly via Athena Health — Elavon. A credit card on file is required. Simple, automatic, and hassle-free.

Terms & Policies
Contract & Cancellation
12 months from enrollment date.
30-day written advance notice required before the next billing cycle.
$100 fee applies if membership is cancelled before the 12-month term is completed.
A valid waiver may be granted for relocation or severe medical conditions with proper written proof submitted to our office.
Credits & Refunds
Use it or lose it — unused credits and benefits do not roll over to the next month.
All unused credits expire immediately upon cancellation. No exceptions.
Past monthly membership payments are strictly non-refundable and cannot be prorated.
Processed monthly via Athena Health — Elavon. A valid credit card on file is required for enrollment.
Freeze / Pause Policy
Members may freeze their membership for 1–3 months within a 12-month contract period (Temporary Suspension).
Monthly auto-draft payments stop. Member-only pricing and service redemption are also suspended for the duration of the freeze.
All benefits and billing resume automatically at the end of the freeze period.
Appointment Policy
24–48 hours advance notice required to cancel or reschedule an appointment.
$75–$150 will be charged for late cancellation or missed appointments (no-shows).
Travel Consult/Vaccination consultation fee applies even for members. All other consultations are waived.
